
Aftercare Service
My Pathways Community Aftercare Service, based at Teach na nDaoine FRC, works to support the health & wellbeing of all care experienced members of the community by offering support and advice in many aspects of their lives.
My Pathways Community Aftercare Service
As part of the My Pathways Programme, our aftercare worker, begins working with young people in the care of Tusla, Child and Family agency, before they turn 18 years of age, helping them to prepare for the important, but often unsettling, transition to independence. By working closely together, our aftercare worker and the care leaver create an aftercare plan; this aftercare plan identifies the personal goals of each care leaver, and the skills assessment process identifies the strengths and skills which will help them achieve these goals. If support is needed in particular areas, our aftercare worker is available to advise, as well as assist the care leaver to make links with other professionals.

Referrals and signposting to other services, such as counselling, Social Prescribing Service etc.

Drop-In Service
My pathways community aftercare service Monaghan, runs a drop-in which works to support all care experienced members of the community. There is no age limit on the drop-In Service; if you have any experience of being in care, the service is here to offer support and advice. The drop-in service is also available to foster carers and other professionals seeking advice and information.
The drop-In service runs every Thursday between 3-5pm, call in to Teach na nDaoine FRC.

Aftercare Newsletter
The community aftercare service publishes a monthly online newsletter which encourages young people, foster carers and professionals to stay up to date with local services, supports and groups of interest. It regularly highlights the achievements and experiences of care experienced people, as well as providing information about aftercare in general.
You can subscribe to the newsletter here

Participation
The community aftercare service endeavours to offer care experienced young people availing of the service opportunities to plan and participate in regular recreational and educational activities, as well as provide feedback on the service.
This is done via aftercare events, regular meetings with young people and online surveys and feedback forms.
